Backerboard and Tub Flanges

I am wondering if a tile backerboard (like Wonderboard or Hardiboard) should go OVER the tub flange, or should it be flush with it (allowing the tile to go directly over the flange)?

Also, you've probably seen a million bathrooms - what is the rule about drywall touching the floor on new construction? It would seem to me that the drywall should be raised up off the subfloor a couple of inches so it doesn't soak up any spills, etc? Your thoughts?
