Basement Bathroom - Concrete floor

I own a recently rennovated older home with a poured concrete foundation and basement floor. The person who rennovated the home left the concrete floor broken open where he hooked the new drainage into the main line. He installed the drain for the toilet and sink in the basement, but I need to replace the concrete. The existing floor seems to be quite thin and set on sand/dirt (no gravel). The area of concrete to replace is the majority of the plan for the bathroom. Should I use a vapor barrier and gravel, or just continue with the existing plan of 2-3" concrete over sand/dirt?
