Good rear-exit toilet and how to install it?

I have a bathroom with a wall-hung toilet. We would like to replace it. I'm not sure I want another wall mount. There is a substantial metal support/mount bracket that supports the toilet, but it seems like there is enough flex in the mounting of this to the wall framing that whenever I install the toilet level, over time with use it ends up sloping downward. A rear-exit toilet could be an alternative, but it seems like it could be somewhat tricky making sure the exit plumbing is precisely lined up with the height of the toilet discharge. I'm not sure how easy it will be to convert the present wall-hung discharge to line up with a rear-exit toilet, and it seems like any future remodels of the floor could screw up a rear-exit toilet installation.

So what options are available for a good residential wall-mount and rear-exit, and what should I anticipate for installation problems?
