custom shower/tub, several questions

I am considering building a custom shower (or shower/
tub) rather than buying a tub or tub/shower combination.
I do not seem to be able to find information on several
issues:

1. In a house with one bathroom, does code require
you to have a bathtub?
2. Is there any material, such as a thin concrete,
or plaster, or trowel-on material which I can use as
the surface of the basin and sides (6-20" walls),
rather than tile?
3. If there is not, then will tile with the proper,
normal underlayments be adequate to form a bathtub,
or is it too much work, weight, etc. to bother?

I'd love to hear from anyone who has done this, whether
it is worth the work, or if it should even be attempted.
