The tub faucet should be repairable without taking off tile. It's a little hard to tell from the description, whether you mean the diverter on the tub spout, which would require replacement, or whether it is the center handle on a three handle set.

The sounds in the upstairs pipes, could be a loose faucet washer on the hot side. This could be a simple fix.
