There's a fitting on top of your hot water heater that connects the tank to the hot water piping. It can build up with scale (possibly iron .. in my house it's always a dark brown color) and eventually reduce the water passage to a pinhole. At this point, you'll notice that water trickles in your washing machine, the scald protection balancers in the shower doesn't work properly, and if you turn 2 taps on at the same time, it drops pressure to almost zero. The symptoms come on pretty fast, although it can take years to accumulate to the pinhole size. You'll probably have to replace the fitting.
