Full bathroom in basement

I have a full basement that appears to have been made ready for a full bathroom. Here's what I have: There are 2 black PVC pipes coming up thru the concrete floor. One is 3" in dia. the other is 3 1/2". These 2 pipes are approx. 16" apart from each other. About 3 ft. from these pipes is a square hole in the floor measuring 12" square, and it is filled with dirt. (this may be for a shower?) I have a 20" dia. hole in the floor, and it is tightly sealed with a heavy steel manhole-type cover. There are 3 2" holes thru the steel cover. This hole is about 2 ft away from my sump pump hole. I'm able to handle the carpentry and framing, but I know nothing about this type of plumbing. Other than a sewage pump, what else is needed to get things up and running? I'm on a well and septic system. Thanks for your suggestions.
