It would tie in just below the washing machine drain inlet. If this is acceptable, how high should the drain in the shower be above the inlet to the main drain.

I would recommend breaking concrete and running the pipe below slab. Stepping out of a shower that high worries me. A vent will need to be in the line before it reaches the washer line. Nothing like a washer dumping it's load to just suck the water out of the p-trap on the shower. Oweee! That could smell. Terry
