Mold & mildew on ceramic? I don't think so, maybe on the grout or in a crevice, but not on the tile itself! Why do you want a towel bar IN the shower anyway? Your options seem to be adding ceramic fixtures after removing a few tiles (I don't think you should mount them on top of the existing tiles) or switch to the 'ol standby "shower caddy". Shower caddies are usually metal (chromed or plastic coated) shelving things that hang on the shower head. They work great (if they eventually rust a little, buy a new one at $15.00!) AND they hold soap, razors and, above all, SHAMPOO BOTTLES.
