Question on dual direction clean-out installation....

Replacing 4 inch main line from cast iron to ABS. The plumber will be installing a clean out outside of the house to clean the sewer line toward the city sewer . From reading in this forum, some of you have installed another outside clean-out installed to clean the sewer line toward the house direction also. Dual clean-outs one pointed toward the street , the other pointed toward the house.
There is no other clean-out in the existing plumbing other than going in through a toilet for a straight shot to the street.
If a clean-out is installed pointing toward the house, help me understand how that would not just push any future obstructions toward the house if a plumber snaked the line in the future.
The house has a crawl space .

The snake has a rotating cutter head...it chops stuff up as it goes, scouring the inside of the pipe. You do need to run water to clear the stuff chopped out, but it should not actually push stuff along in the process.
