OK. The toilet is installed and flushes great leaving clean bowl (sometimes you have to flush again). The issue with the first one was, as expected, faulty cut hole(s). I spent $500 to have a plumber replace PRV, but as Terry (and SLOAN flushmate website from the link Terry posted) correctly predicted it didn't help with the toilet. Ordered another one and used the bowl from the second one.
I am incredibly glad that the bowl was bad. Just chasing the issue and fixing the water pressure (from 103 psi to 75 psi) solved a whole bunch of problems in my house that for many years I attributed to poor quality of appliances.
Special thanks to Terry (and SLOAN/Kohler support) for encouraging me to keep on going.
I ordered two more to install in the other bathroom, so that I could build one, in case the bowl is poorly cut again.
