Possible reasons a mixet valve would shoot out of the socket?

    I had this fixed a while ago, but didn't get an explanation why it happened.
    Tenant told me the valve and knob shot out of the wall for no reason.
    I measured the water pressure to the house and it's 70psi.

    What could cause the valve to shoot out like this?

    The "ridge" on the locknut must have deteriorated because it holds the spacer washers and stem in place.
