There are still some plumbing hacks out there that don't know how to plumb for back to back toilets.
Using the double santee fitting below, allows a flushed toilet to push water out of the opposing bowl. The new toilets using a 3" flushvalve and even those using smaller valves, but tuned for quicker flushing will push water out in this situation. If you read the instuctions that come with the toilets, you will see the warning.

It seems there are plumbers out there that don't like reading, so the instructions get tossed. Many inspectors don't understand the new products, so the same mistakes are built into new homes.
Even the double fixture cross has the issue of allowing water from one flush to enter the arm leading to the next bowl. When this happens, water rises in the bowl, and then sloshes out. After a few flushes in one bathroom, the bowl in the lesser used room is neary emptied.

If you don't have room for a double wye, and you wye off lower in the wall, and come up with two santees, one for each toilet. the vents can then revent above flood level on the next floor.
