I'm not sure what to make of this. How do I attach a tub spout to this? Can it be done effectively?

You will need to open up the wall and replace with either copper all the way to the valve, or threaded nipples made of brass or galvanized.
That is undersized CPVC.
Because the pipe is undersized, it can force water up to the shower head while filling the tub.
There is also no good way to attach a spout to that.

It could be CPVC. What's on the other side of that wall? If you're lucky, it might be a closet and you can go in from there to potentially rework things. I would not be real confident about gluing something to the plastic pipe to make a connection, but I'm not a pro.
