Need Advice for Maximum slope and clean out location

Please help me for the maximum slope for a drain and where to put clean out. Attached here is a plumbing layout of my washroom and kitchen.
Question A: Is there a minimum length before the wye?
Question B; Is there a maximum slope for a drain?
C: Please help where is the proper place to put my clean out

Any suggestions, corrections or comments are highly appreciated.
