Cleanout question for 3" bathroom drain

Please see the attached images. The 3" PVC/DWV coming down into my basement is a 3" drain from a 2nd story bathroom (2 sinks, 1 toilet, 1 shower). I'm getting this inspected later this week and would like to pass the first time. My questions are in regard to the cleanout wye.

1) Will the cleanout I installed pass inspection... does it meet code?
2) Is a cleanout even required?
3) Would I be better off installing the cleanout in the vertical line above the basement floor? If so, can I cover the cleanout with drywall or do I need to provide access to it?

All the other drains in my basement have their own cleanout, but they're on the opposite side of where the vertical pipe transitions to horizontal when compared to the one in question here. Obviously because I'm so close to a wall, I couldn't do it this way.

Also, I've since added a strap/hanger to the pipe at the last floor joist.

The problem with the basement c/o is when the line is clogged you are going to have fun opening that c/o plug and flooding the basement,
that is one reason besides code to put the c/o in upstairs, but you do not cover
the c/o it needs to accessible otherwise what is the use of installing the c/o,
again when the line is back up you do not want to have to pull the toilet
this from a master plumber with over 30 years experience snaking drains
